1. Download this Verification of Attendance form.

2. Open it in any PDF viewing software (Adobe, Preview, etc.).

3. Fill in your name and the date by running your cursor over the two remaining blanks on the form and putting your info in the editable box that pops up (you’re welcome!).

4. Pick your CMH/CEU tracking method:

A. Manually: Keep the Verification of Attendance form in your records and add it to your other CMHs/CEUs when you submit your Certification Maintenance Compliance form at the end of your three-year certification maintenance interval.

B. Online ASHA CE Registry: This is an optional service that tracks your CMHs/CEUs for you online. My course is not affiliated with the ASHA CE Registry. Although the hours you earned from this course won’t show up in your online CE Registry, you still earned hours for this course. Simply add them to the sum of your other CMHs/CEUs on your CE Registry when you submit your Certification Maintenance Compliance form at the end of your three-year certification maintenance interval.

5. Keep this Verification of Attendance form in your records for procurement if needed if you’re audited.
